Click on the above link to view and print our Library Card Application. Fill in the required fields and return the completed form to the Library Circulation Desk along with your proof of Massachusetts residency. This can be a driver's license with your current Massachusetts address or a picture ID in combination with a voter registration card, check book, bank book, bill, rental agreement or some other document with your current Massachusetts address. Library cards are free for Massachusetts residents. Non-residents will need to pay a $5.00 fee with their application to be eligible for library privileges at the Mashpee Public Library.
